Diese Methode zielt darauf ab, eine Befruchtung zu erreichen, indem die Spermien direkt in die Eizelle injiziert werden, wenn die Spermien keine normale Befruchtungsf\u00e4higkeit besitzen. IVF und andere bekannte Behandlungsmethoden zeigen bei schwerer m\u00e4nnlicher Unfruchtbarkeit niedrige oder keine Befruchtungsraten. Bei niedriger Spermienzahl, eingeschr\u00e4nkter Beweglichkeit oder morphologischen Defekten ist eine nat\u00fcrliche Befruchtung der Eizelle durch die Spermien nicht m\u00f6glich. Bei der Mikroinjektion wird unter dem Mikroskop das Spermium mittels Mikropipetten in die Eizelle injiziert, die von der Frau entnommen wurde. Die Verfahren erfolgen unter dem Mikroskop bei 200\u2013400-facher Vergr\u00f6\u00dferung auf einer beheizten Oberfl\u00e4che und mit einem Mikromanipulator.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"i-2\"><\/span>F\u00fcr wen ist die Mikroinjektion geeignet?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Die Mikroinjektion kann auch bei Paaren angewendet werden, bei denen keine Befruchtung mit IVF erreicht werden kann oder bei denen nicht gen\u00fcgend Spermien und\/oder Eizellen von ausreichender Qualit\u00e4t f\u00fcr eine IVF vorhanden sind. Das Verfahren kann mit Samen mit sehr geringer Spermienzahl durchgef\u00fchrt werden, oder in F\u00e4llen, in denen im Ejakulat keine Spermien gefunden werden, kann Spermienmaterial aus den Hoden verwendet werden. Bei einigen M\u00e4nnern k\u00f6nnen im Hoden keine Spermien gefunden werden. In solchen F\u00e4llen k\u00f6nnen, falls vorhanden, Spermatiden, die unreifen Zellen, aus denen sich Spermien entwickeln, verwendet werden. Die Ergebnisse mit Spermatiden sind jedoch noch nicht vollst\u00e4ndig erfolgreich, und die Befruchtungsraten sind niedrig.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"i-3\"><\/span>Wie wird die Mikroinjektion durchgef\u00fchrt?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Der Mikroinjektionsvorgang wird unter einem mit spezieller Ausr\u00fcstung ausgestatteten Mikroskop mit sterilen, Einweg-Glas-Mikropipetten durchgef\u00fchrt. Die nach der Eizellentnahme gewonnenen Eizellen werden von den umliegenden Cumuluszellen gereinigt, um reife und verwendbare Eizellen zu identifizieren. F\u00fcr den Mikroinjektionsprozess wird in einer sterilen, ungiftigen Plastikschale ein spezielles Kulturmedium mit den Spermien des Mannes vorbereitet. Anschlie\u00dfend werden Mikrotropfen mit speziellem Kulturmedium f\u00fcr die reifen Eizellen hergestellt. Das Spermium wird immobilisiert, indem sein Mittelst\u00fcck mit der Mikroinjektionspipette gedr\u00fcckt wird. Das immobilisierte Spermium wird in die Pipette aufgenommen und in den Tropfen mit der Eizelle injiziert. Dieses Verfahren wird nacheinander bei allen Eizellen angewendet. Nach Abschluss werden die Eizellen in spezielle Kulturmedien \u00fcberf\u00fchrt und nach 18\u201320 Stunden wird die Befruchtung \u00fcberpr\u00fcft.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"i-4\"><\/span>In welchen F\u00e4llen wird die Mikroinjektion angewendet?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Bei schwerer m\u00e4nnlicher Unfruchtbarkeit fehlen den Spermien die F\u00e4higkeiten zur Befruchtung der Eizelle. Schwere m\u00e4nnliche Unfruchtbarkeit wird definiert als weniger als eine Million Spermien mit schneller progressiver Bewegung und normaler Morphologie. Selbst wenn sowohl Spermien als auch Eizellen v\u00f6llig normal sind, kann es bei der IVF-Methode zu keiner Befruchtung kommen. Bei unerkl\u00e4rter Unfruchtbarkeit erfolgt in 15-20 % der IVF-Versuche keine Befruchtung. Wenn IVF direkt bei Paaren mit normalen Spermienparametern angewendet wird, besteht die M\u00f6glichkeit des Befruchtungsversagens. Deshalb werden bei Paaren mit ausreichender Eizellanzahl (10 oder mehr) Mikroinjektion und IVF gemeinsam verwendet. So werden die Eizellen in zwei Gruppen aufgeteilt: Die Befruchtung wird mittels IVF \u00fcberpr\u00fcft und die Mikroinjektion dient als Vorsichtsma\u00dfnahme gegen Befruchtungsversagen.<\/p>\n<p>Obwohl ICSI heute eine vielversprechende Behandlungsoption bei m\u00e4nnlicher Unfruchtbarkeit darstellt, sind die wichtigsten Faktoren, die den Erfolg beeinflussen, das Alter der Frau und die Qualit\u00e4t der entnommenen Eizellen. Faktoren, die durch fortgeschrittenes m\u00fctterliches Alter und erh\u00f6hte Chromosomenanomalien verursacht werden, beeinflussen den Behandlungserfolg bei m\u00e4nnlicher Unfruchtbarkeit in Abh\u00e4ngigkeit von der Frau.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"i-5\"><\/span>Was ist Embryo-Entwicklungs\u00fcberwachung und wie wird sie durchgef\u00fchrt?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Die Sicherstellung der Embryonalentwicklung ist im Rahmen der IVF-Behandlung der wichtigste Schritt unter Laborbedingungen. Nach der Befruchtung wird die Embryonalentwicklung sorgf\u00e4ltig unter Laborbedingungen und unter Anleitung eines erfahrenen Teams \u00fcberwacht, und der Transfer erfolgt zum geeigneten Zeitpunkt.<\/p>\n<p class=\"translation-block\"><strong>Befruchtung:<\/strong> Die Befruchtung bezeichnet die Verschmelzung einer Samenzelle mit einer Eizelle. Dieser Prozess erfolgt, wenn Spermien die \u00e4u\u00dfere Membran der Eizelle durchdringen und das Zytoplasma der Eizelle erreichen. Als Ergebnis der Befruchtung entsteht ein einkerniger Embryo, der als Zygote bezeichnet wird.<\/p>\n<p class=\"translation-block\"><strong>Zellteilung:<\/strong> Die Zygote teilt sich, um mehr Zellen zu bilden. Zuerst teilt sich die Zygote in einen Zwei-Zell-Embryo, danach setzen sich die Zellteilungen fort. Diese Teilungen f\u00fchren zur Bildung eines mehrzelligen Embryos. Die erste Teilung erfolgt meist etwa 24 Stunden sp\u00e4ter, gefolgt von Zellteilungen in regelm\u00e4\u00dfigen Abst\u00e4nden.<\/p>\n<p class=\"translation-block\"><strong>Morphologische Ver\u00e4nderungen:<\/strong> Der Embryo durchl\u00e4uft neben den Zellteilungen morphologische Ver\u00e4nderungen. Zwischen den Zellen bildet sich eine regelm\u00e4\u00dfige Anordnung, und der Embryo teilt sich in Zellgruppen, die Blastomere genannt werden. Gleichzeitig wird der Embryo zu einer Morula, bestehend aus kleinen Zellen, die durch mitotische Teilungen gebildet werden.<\/p>\n<p class=\"translation-block\"><strong>Blastozystenbildung:<\/strong> In den sp\u00e4teren Stadien der fr\u00fchen Embryonalentwicklung verwandelt sich die Morula in einen Blastozysten. Der Blastozyst enth\u00e4lt eine innere Zellmasse (ICM) und eine Zellschicht, die Trophoblast genannt wird. Die innere Zellmasse dient als Quelle der Zellen, die in sp\u00e4teren Stadien verschiedene Organe und Gewebe des Embryos bilden, w\u00e4hrend der Trophoblast sich in Zellen entwickelt, die die Plazenta und unterst\u00fctzende Gewebe in den fr\u00fchen Schwangerschaftsphasen bilden.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"i-6\"><\/span>Mikroinjektion oder IVF?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Bei der Mikroinjektion sind die Chancen auf Schwangerschaft und Befruchtung im Vergleich zur klassischen IVF h\u00f6her. Die Erfolgsraten variieren jedoch je nach Zustand des Paares.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"i-7\"><\/span>Wann wird die Mikroinjektions-Injektion durchgef\u00fchrt?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Die Mikroinjektion wird durchgef\u00fchrt, nachdem die Eizellen nach der Eizellentnahme vorbereitet Every consult he made me feel valued as a patient and answered all my questions and doubts. As an international patient, I had a lot of questions, but he put me at ease.\n\nWe decided to move with Dr. Soner on our journey. He studied my case and made changes as needed after every ultrasound check. The staff, Ms. Hazel was also wonderful. I could ask a question whenever during the process, and I was comforted that they cared to answer. There was a bit of a hiccup in my journey, unplanned situation during my IVF procedure, but nevertheless, we went with the flow and trusted Dr. Soner. We returned back home, and after a couple of months of the procedure, we proceeded with a frozen transfer, we are now pregnant with our first baby! This is after 1.5 years of unexplained fertility.\n\nNow, we are back patiently waiting for\u00a0 our child. As I reflect back, I feel extremely grateful that we chose Dr. Soner and he chose us as patients, too.
