Diese Methode ist ideal bei wiederholtem Schwangerschaftsverlust, wiederholtem Implantationsversagen beim IVF, schwerer m\u00e4nnlicher Infertilit\u00e4t, genetischen Ursachen wie Translokationstr\u00e4gern oder zur Verk\u00fcrzung der Zeit bis zur Schwangerschaft durch Auswahl der Embryonen. Der Vorgang erfolgt durch eine Biopsie des Embryos.<\/p>\n<p>Wenn der Embryo den 5. oder 6. Tag erreicht, wird er Blastozyste genannt. Nach Entnahme von 5-10 Zellen aus dem Trophektoderm, das sp\u00e4ter zur Plazenta wird, wird er im Genetiklabor mittels Next-Generation-Sequencing (NGS) analysiert, um die Anzahl und Struktur der Chromosomen zu bestimmen. Die NGS-Technologie ist im Vergleich zur \u00e4lteren Array-CGH-Methode empfindlicher.<\/p>\n<p>Mit dieser Methode k\u00f6nnen Mosaik-Embryonen mit einem Anteil zwischen 20 % und 80 % festgestellt werden.<\/p>\n<p>Normal (euploider Embryo): Ein Embryo gilt als normal, wenn er von jedem Chromosom zwei Kopien hat und die Geschlechtschromosomen XX oder XY sind.<\/p>\n<p class=\"translation-block\"><strong>Anormal (aneuploid Embryo):<\/strong> Ein Embryo wird als abnormal bezeichnet, wenn er von einem Chromosom eine Kopie, drei oder mehr Kopien hat oder wenn numerische oder strukturelle Abnormalit\u00e4ten in den Geschlechtschromosomen vorliegen.<\/p>\n<p class=\"translation-block\"><strong>Mosaik-Embryo:<\/strong> Manchmal sollte die Anzahl der Kopien in einem oder mehreren Chromosomen eines Embryos 2 sein, aber sie kann in einem Bereich zwischen 1-2 oder 2-3 liegen. Diese Zwischenwerte werden prozentual ausgedr\u00fcckt und als Mosaizismus im Embryo definiert. Die Entscheidung zur Bewertung dieses Embryos wird basierend auf dem Prozentsatz des Mosaizismus sowie der Krankengeschichte und dem Zustand des Patienten diskutiert.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"i-2\"><\/span>Pr\u00e4implantationsgenetischer Test \u2013 Strukturelle Umlagerungen (PGT-SR)<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Auch wenn die Mutter oder der Vater gesund erscheint, kann eine vorhandene chromosomale Abweichung negativ auf die Embryonen \u00fcbertragen werden. Eine solche Situation ist die \u201eTranslokation\u201c. Es gibt zwei verschiedene Arten von Translokationen: reziproke, bei der zwei oder mehr Chromosomen Segmente miteinander austauschen, und robertsonsche, bei der zwei Chromosomen end-zu-end verschmolzen sind und als ein einziges Chromosom erscheinen.<\/p>\n<p>Eine Person mit einer balancierten Translokation kann v\u00f6llig gesund sein, aber wenn in ihrem Ei oder Sperma eine unausgewogene Translokation vorliegt, kann diese als chromosomale Anomalie auf den Embryo \u00fcbertragen werden. Dies kann eine Schwangerschaft verhindern oder zu Fehlgeburten f\u00fchren.<\/p>\n<p>Je nach Gr\u00f6\u00dfe der ausgetauschten Chromosomensegmente oder der Region, in der sie auf dem Chromosom liegen, kann eine Analyse mit Next Generation Sequencing (NGS) durchgef\u00fchrt werden. Der Anteil der Translokationstr\u00e4ger steigt bei Paaren mit wiederholtem Implantationsversagen auf bis zu 3 % und bei Paaren mit wiederholten Fehlgeburten auf bis zu 10 %.<\/p>\n<p>Die Erkennung abnormaler Embryonen durch pr\u00e4implantationsgenetische Diagnostik und die \u00dcbertragung normaler oder balancierter Embryonen verbessert die Schwangerschaftserfolge erheblich.<\/p>\n<p>Obwohl die Chance, einen normalen oder balancierten Embryo zu finden, gering ist, sind die Schwangerschaftsraten bei Behandlungen mit Transfer ziemlich hoch. PGT erh\u00f6ht die Schwangerschaftsraten und reduziert Schwangerschaftsverluste in dieser Patientengruppe.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"i-3\"><\/span>Pr\u00e4implantationsgenetischer Test \u2013 Einzelerkrankungen<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Es gibt Krankheiten mit famili\u00e4rer genetischer Vererbung. Die genetische Untersuchung von Embryonen mittels IVF verhindert die Weitergabe an nachfolgende Generationen und tr\u00e4gt auch zur Entstehung einer gesunden Schwangerschaft bei.<\/p>\n<p>Das Verfahren, das durchgef\u00fchrt wird, um dies zu verstehen, wird als PGT-M bezeichnet. Im ersten Schritt dieser Behandlung wird die Region der genetischen Mutation identifiziert. Anschlie\u00dfend werden Biopsien von Embryonen, die durch IVF gewonnen wurden, im Genetiklabor sowohl auf die genetische Mutation als auch auf numerische und strukturelle Chromosomenanomalien untersucht. Dadurch wird sichergestellt, dass ein genetisch gesunder Embryo \u00fcbertragen wird und der Embryo chromosomal normal ist.<\/p>\n<p class=\"translation-block\">Es wird Paaren empfohlen, die eine genetische Krankheit haben oder als Tr\u00e4ger identifiziert wurden und gesunde Kinder haben m\u00f6chten. Da das Risiko f\u00fcr Krankheiten bei Kindern von Paaren mit Einzelgenst\u00f6rungen oder Chromosomenanomalien hoch ist, wird es heutzutage h\u00e4ufig Every consult he made me feel valued as a patient and answered all my questions and doubts. As an international patient, I had a lot of questions, but he put me at ease.\n\nWe decided to move with Dr. Soner on our journey. He studied my case and made changes as needed after every ultrasound check. The staff, Ms. Hazel was also wonderful. I could ask a question whenever during the process, and I was comforted that they cared to answer. There was a bit of a hiccup in my journey, unplanned situation during my IVF procedure, but nevertheless, we went with the flow and trusted Dr. Soner. We returned back home, and after a couple of months of the procedure, we proceeded with a frozen transfer, we are now pregnant with our first baby! This is after 1.5 years of unexplained fertility.\n\nNow, we are back patiently waiting for\u00a0 our child. As I reflect back, I feel extremely grateful that we chose Dr. Soner and he chose us as patients, too.
