TOT surgery for urinary incontinence is a minimally invasive surgical method applied to women who experience stress-type urinary incontinence. Urinary incontinence \u2013 loss of bladder control \u2013 is one of the common problems that can be seen at all ages. Its severity ranges from occasional leakage when you cough or sneeze to a sudden and strong urge to urinate that is so intense you may not reach the toilet in time.<\/p>\n

Although urinary incontinence becomes more common as people age, it is not an inevitable result of aging. If urinary incontinence affects your daily activities, do not hesitate to see your doctor. For most people, simple lifestyle and dietary changes or medical care can treat the symptoms of urinary incontinence.<\/p>\n

<\/span>Types of Urinary Incontinence<\/span><\/h2>\n

Types of urinary incontinence include:<\/p>\n

Stress incontinence:<\/strong> Urine leaks when you put pressure on your bladder by coughing, sneezing, laughing, exercising, or lifting something heavy.<\/p>\n

Urge incontinence:<\/strong> You experience involuntary urine loss following a sudden, intense urge to urinate. It is the condition in which you need to urinate frequently, including throughout the night. Urinary incontinence may be caused by a minor issue such as an infection or a more serious condition such as a neurological disorder or diabetes.<\/p>\n

Overflow incontinence:<\/strong> You experience frequent or constant dribbling of urine due to a bladder that does not empty completely.<\/p>\n

Functional incontinence:<\/strong> A physical or mental impairment prevents you from getting to the toilet in time. For example, if you have severe arthritis, you may not be able to unbutton your pants quickly enough.<\/p>\n

Mixed incontinence:<\/strong> You experience more than one type of urinary incontinence \u2013 most often this refers to a combination of stress incontinence and urge incontinence.<\/p>\n

<\/span>When Should You See a Doctor?<\/span><\/h2>\n

You may feel uncomfortable talking to your doctor about urinary incontinence. However, if urinary incontinence is frequent or affects your quality of life, it is important to seek medical help because urinary incontinence:<\/p>\n

    \n
  • Causes you to restrict your daily activities and limit your social interactions.<\/li>\n
  • Negatively affects your quality of life.<\/li>\n
  • Increases the risk of falls in older adults when rushing to the toilet.<\/li>\n
  • An underlying, more serious condition may be overlooked.<\/li>\n<\/ul>\n

    <\/span>How Is Urinary Incontinence Diagnosed?<\/span><\/h2>\n

    It is important to determine the type of urinary incontinence you have, and your symptoms often tell your doctor which type it is. This information will guide treatment decisions. A comprehensive history and physical examination are started, and then you may be asked to perform a simple maneuver, such as coughing, that can demonstrate incontinence.<\/p>\n

    After this, the recommendations are:\nUrinalysis: A sample of your urine is checked for signs of infection, traces of blood, or other abnormalities.<\/p>\n

    Bladder diary: Over several days, you record how much you drink, when you urinate, the amount of urine you produce, whether you feel the urge to urinate, and the number of incontinence episodes.<\/p>\n

    Ultrasound check after you empty your bladder: You are asked to urinate into a container that measures urine output. Then, an ultrasound test is used to check the amount of urine remaining in your bladder. Having a large amount of residual urine in your bladder may indicate a blockage in your urinary tract or a problem with the nerves or muscles of your bladder.<\/p>\n

    <\/span>Urinary Incontinence Treatment<\/span><\/h2>\n

    Behavioral techniques:<\/strong> Bladder training is recommended to delay urination after you get the urge to go. You can start by trying to wait 10 minutes every time you feel the urge to urinate. The goal is to lengthen the time between toilet visits until you urinate only every 2.5 to 3.5 hours.\nDouble voiding is recommended to help you learn to empty your bladder more completely to prevent overflow incontinence. Double voiding means urinating, then waiting a few minutes, and trying again.<\/p>\n

    Scheduled toilet visits are recommended, meaning urinating every two to four hours instead of waiting for the urge to go.\nFluid and diet management to regain control of your bladder. You may need to reduce or avoid alcohol, caffeine, or acidic foods. Reducing fluid intake, losing weight, or increasing physical activity may also help ease the problem.<\/p>\n

    Pelvic floor muscle exercises:<\/strong> You are advised to perform these exercises frequently to strengthen the muscles that help control urination. These techniques, also known as Kegel exercises, are especially effective for stress incontinence but can also help reduce urge incontinence.<\/p>\n

    To perform pelvic floor muscle exercises, imagine that you are trying to stop your urine flow. At those times:<\/p>\n

      \n
    • Tighten (contract) the muscles you would use to stop urinating and hold for five seconds, then relax for five seconds. (If this is too difficult, start by holding for two seconds and relaxing for three seconds.)<\/li>\n
    • Try to hold the contractions for 10 seconds at a time.<\/li>\n
    • Aim for at least three sets of 10 repetitions each day.<\/li>\n<\/ul>\n

      We may recommend that you work with a pelvic floor physical therapist or try biofeedback techniques to help you identify and contract the correct muscles.\nThere are two types of devices designed to treat women with incontinence: A urethral insert is a small, tampon-like, single-use device inserted into the urethra before a specific activity, such as exercise, that may trigger urinary leakage.<\/p>\n

      The insert acts as a plug to prevent leakage and is removed before urination.\nA pessary is a flexible silicone ring that you place in your vagina and wear throughout the day. The device is also used in women with vaginal prolapse. The pessary helps support the urethra to prevent urine leakage.<\/p>\n

      <\/span>Interventional Treatments<\/span><\/h2>\n

      Interventional treatments that may help in the management of incontinence include:\nSynthetic material injections: A synthetic material is injected into the tissue surrounding the urethra. This material helps keep the urethra closed and reduces urine leakage. This procedure is for the treatment of stress incontinence and is generally less effective than more invasive treatments such as surgery. It may need to be repeated more than once.<\/p>\n

      OnabotulinumtoxinA (Botox):<\/strong> Injections of Botox into the bladder muscle may benefit people who have an overactive bladder that causes urinary incontinence. Botox is usually preferred when other treatments have not been successful.<\/p>\n

      Nerve stimulation:<\/strong> Painless electrical impulses may be applied to stimulate the nerves (sacral nerves) involved in bladder control.<\/p>\n

      <\/span>Urinary Incontinence TOT Surgery<\/span><\/h2>\n

      If other treatments do not work, several surgical procedures can treat the problems that cause urinary incontinence:<\/p>\n

      Sling procedures:<\/strong> Your body's tissue, synthetic material (mesh), or strips are used to create a pelvic sling under your urethra and the thickened muscle area where the bladder connects to the urethra (bladder neck). The sling helps keep the urethra closed, especially when you cough or sneeze. This procedure is used to treat stress incontinence.<\/p>\n

      Bladder neck suspension:<\/strong> This procedure is designed to provide support to your urethra and the bladder neck, which is a thickened muscle area where the bladder connects to the urethra. It involves an abdominal incision, so it is performed under general or spinal anesthesia.<\/p>\n

      Prolapse surgery:<\/strong> In women with pelvic organ prolapse and mixed incontinence, surgery may involve a combination of a sling procedure and prolapse surgery. Repairing pelvic organ prolapse alone does not routinely improve urinary incontinence symptoms.<\/p>\n

      Artificial urinary sphincter:<\/strong> A small, fluid-filled ring is implanted around the bladder neck to keep the urinary sphincter closed until you feel the need to urinate. To urinate, you press a valve implanted under your skin, which causes the ring to deflate and allows urine to flow from your bladder.<\/p>\n\n\t\t<\/div>\n\t<\/div><\/div><\/div><\/div><\/div><\/div>
