What is Chocolate Cyst and What Are Its Symptoms?

A chocolate cyst is a condition caused by the growth of the uterine lining (endometrium), also known as endometriosis, outside the uterus. These cysts can be found around the ovaries, intestines, bladder, and other internal organs. Chocolate cysts grow at the beginning of the menstrual cycle and may cause bleeding. The blood inside the cyst thickens over time and takes on a “chocolate-like” appearance, which is why these cysts are called “chocolate cysts.” Symptoms of chocolate cysts can include pain, cramps, bleeding during menstruation, and pain during sexual intercourse. Additionally, depending on the size and location of the cysts, they may press on the intestines or bladder, causing difficulty with urination or bowel movements. Chocolate cysts can also cause infertility problems. They can affect the function of the ovaries and make it harder to become pregnant. Treatment varies depending on the severity of symptoms and the patient's age. For patients with mild symptoms, painkillers and hormone therapy may be recommended. For patients with more severe symptoms, laparoscopic surgery may be preferred to remove the cysts. What Causes Chocolate Cyst?

Chocolate cysts are caused by a condition called endometriosis, which is the growth of the uterine lining (endometrium) outside the uterus. Endometriosis is the growth of tissue that normally forms inside the uterus during the menstrual cycle, but grows outside the uterus. Chocolate cysts form as this growing endometrial tissue accumulates around the ovaries, intestines, bladder, and other internal organs. Unlike the normal uterine lining, this tissue causes bleeding during the menstrual cycle, so blood collects inside the cysts and thickens over time, taking on a "chocolate" consistency. The exact cause of chocolate cysts is not fully known. However, certain factors are believed to contribute to their formation. These include genetic predisposition, immune system problems, hormonal imbalances, and environmental factors. Additionally, chocolate cysts can form due to backward flow of menstrual blood during the period. This condition is called "retrograde menstruation" and can cause the cysts to grow.

How is Endometriosis Chocolate Cyst Diagnosed?

Various methods can be used to diagnose endometriosis chocolate cysts. However, the most definitive diagnostic method is laparoscopy. Laparoscopy is a minimally invasive surgical procedure that allows viewing inside the uterus, fallopian tubes, and ovaries with the help of a camera. During this procedure, the presence and location of cysts can be seen, and a biopsy can be taken to determine whether the cysts are caused by endometriosis. Another diagnostic method that can be preferred is ultrasonography (USG). USG provides images of the uterus, fallopian tubes, and ovaries. Endometriosis chocolate cysts show a characteristic appearance on USG images. However, it is not as definitive a diagnostic method as laparoscopy. Magnetic resonance imaging (MRI) can also be used to diagnose endometriosis chocolate cysts. MRI produces sectional images of the body and can provide detailed information about the size, location, and other features of the cysts. Blood tests can also be helpful in diagnosing endometriosis chocolate cysts. However, some blood tests can be used to determine whether the cysts are caused by endometriosis.

How is Endometriosis (Chocolate Cyst) Treated?

Endometriosis treatment may vary depending on the severity of symptoms and the patient’s age. The goal of treatment is to control symptoms and reduce the risk of infertility. The methods we use in endometriosis treatment are as follows:

  • Painkillers: Painkillers can be recommended for patients with mild symptoms. These medications relieve pain and help control symptoms.
  • Hormone therapy: Hormone therapy is used to regulate the menstrual cycle and control symptoms. Hormone therapy can be provided with medications such as birth control pills, progesterone-containing drugs, GnRH agonists, and danazol.
  • Laparoscopic surgery: Laparoscopic surgery may be performed on patients with more severe symptoms. During this procedure, the cysts are removed, endometriosis tissue is cleared, and damaged tissues are repaired. After the surgery, the patient's recovery process and symptoms are monitored.
  • In vitro fertilization (IVF) treatment: Endometriosis chocolate cysts can cause infertility problems. In such cases, IVF treatment may be recommended. IVF treatment involves stages such as ovarian stimulation, egg retrieval, and fertilization following laparoscopy.
  • A correct diagnosis before treatment is extremely important for appropriate treatment planning and reducing the risk of infertility. Pregnancy After Chocolate Cyst Surgery

Pregnancy after chocolate cyst surgery is possible but not always guaranteed. Removal of the cysts and clearing of endometriosis tissue can increase the chance of natural conception. In the postoperative period, depending on the healing process and the patient's symptoms, a certain waiting time may be required before trying to conceive. Doctors generally recommend waiting at least 6 months. Additionally, infertility problems may persist after chocolate cyst surgery. In such cases, assisted reproductive techniques like IVF can increase the chance of pregnancy.

Does Chocolate Cyst Prevent Pregnancy?

Chocolate cysts can cause infertility problems and make it difficult to become pregnant. The cysts may affect ovarian functions and interfere with the ovulation process. However, the presence of cysts does not definitively prevent pregnancy. Some patients can conceive naturally despite having chocolate cysts or may try IVF treatment.
