Why Are Sperm Preparation Techniques Used?

Sperm preparation methods are commonly used in couples undergoing infertility treatment, especially in assisted reproductive techniques such as in vitro fertilization (IVF) and intrauterine insemination (IUI). Here are the general purposes of these techniques:

Selecting the Best Quality Sperm Cells: Sperm preparation techniques select sperm based on characteristics such as motility, morphology (shape and structure), and DNA integrity. This increases treatment success by enhancing the chances of fertilization and the development of a healthy embryo through the selection of the healthiest and highest-quality sperm.

Separating Sperm from Seminal Plasma: Seminal plasma is the fluid that protects and carries sperm, but it can sometimes contain harmful components (such as free radicals or bacteria). Sperm preparation techniques separate sperm from these potentially harmful substances.

Activating Sperm: The sperm preparation process also enhances sperm motility and prepares them for the fertilization stage.

Removing Damaged or Dead Sperm Cells: Some sperm preparation techniques separate damaged or dead sperm cells. This helps reduce the risk of genetic abnormalities and increases the likelihood of healthy embryo development.

Does Sperm Washing Increase the Success Rate of IVF?

The purpose of sperm washing is to select the healthiest and most motile sperm to increase the chances of fertilization. Sperm washing separates sperm from the seminal plasma, removing potentially harmful components as well as less motile or abnormal sperm. This can improve the likelihood of fertilization and enhance the chances of healthy embryo development.

Therefore, sperm washing can increase the success rate of IVF or IUI. However, sperm washing alone does not guarantee the success of the treatment. The success rate of IVF also depends on various factors such as the woman's age, overall health, ovarian reserve, and sperm quality.

What Are the Sperm Preparation Techniques?

Sperm preparation techniques allow the selection and use of the highest quality and healthiest sperm cells in couples undergoing IVF treatment. The most common sperm preparation methods are:

  1. .Simple Wash: In this method, semen is centrifuged to separate the sperm from the seminal plasma. The sperm are then washed in a special culture medium and prepared for fertilization.
  2. Density Gradient Technique: In this method, semen is centrifuged with special solutions of different densities. Since the highest quality sperm are the heaviest, they separate from other cells and seminal plasma and accumulate in the bottom layer.
  3. Swim-Up Technique: In this technique, sperm are placed in a culture medium, and the most motile sperm are expected to "swim up" to the surface. This method allows for the selection of the best sperm in terms of motility and morphology.
  4. Electrophoresis: Although this is a newer and less common technique, it separates sperm based on their motility speed. An electric field is used, and the healthiest sperm are generally the fastest moving ones.
  5. MACS (Magnetic Activated Cell Sorting): Sperm are labeled with microbeads that bind to markers of apoptosis (programmed cell death). They are then passed through a magnetic field, and the labeled sperm are removed. This allows for the selection of healthier sperm with higher fertilization potential.
  6. Mini Gradient Technique: This method is especially used in men with low sperm count. It works similarly to the Density Gradient technique but requires a smaller amount of semen. Two or more special solutions with different densities are used. These solutions are layered into a tube, each arranged to form a distinct density layer. The semen sample is placed on the top layer, and the tube is then centrifuged. During centrifugation, sperm cells separate based on their densities. The healthiest and most motile sperm, being the heaviest, move to the bottom layer, separating from other cells and seminal plasma. This bottom layer is then collected and used for reproductive procedures.
